1.In this screen, user need to add all data

• First name – It is mandatory to add , Middle name is not mandatory and last name is mandatory
• Image is not mandatory to be added
• City – will be added manually
• State – will be selected from the dropdown
• Zipcode will be added manually
• Height – will be added manually in this format “ 5’ 7” “
• Weight – Will be added in pounds manually
• Gender – It will be selected from dropdown
• Ethnicity – will describe the ethnicity of patient (Selected from dropdown)
• Date of Birth – will be added by calendar or manually
• Martial status – Will be added from dropdown
• After filling all these details click on save , it will redirect user to “Allergies” page

2. Allergies – After building profile and on click of save it will redirect patient to Allergies page

• Patient can skip this too
• User can also add data if he/she wants to
• Reaction to medications – Drug name , Reaction/ comment – In this patient can also add multiple custom fields. By clicking on “Add more”
• Food Allergies – Food name , Reaction/ comment – In this patient can also add multiple custom fields. By clicking on “Add more”
• On filling values and saving it or on click of skip – It will redirect user to next screen “Medical history”

3. Medical History – In this user can add, medical conditions he/she suffered earlier and can also skip it

• It is multiple checkbox where user can select multiple medical conditions and on click of save it will store data
• On click of “Skip” it will redirect user to next screen “Surgeries”

4. Surgeries – This screen will be used to add the name of surgeries happened before and on which year it happened + patient age when surgery happened

• On click of skip it will redirect patient to “medications” screen

5. Medications – this screen will appear to add prescribed medicines which Patient is using earlier

• Patient can add multiple medications too by clicking on “Add more”
• Patient can also skip the medication ,then it will redirect user to “Sexual Health” screen

6. Sexual Health – This screen will appear after Medications screen

• It is single selective checkboxes (user can only select one)
• Patient can skip it too and select checkbox and save it, This will redirect user to “Health Habits” page

7. Health Habits – This screen will appear after sexual health screen. In this, patient needs to add his/her health habits

• It is single selective checkbox – User can only select one checkbox and on click of save it will store details
• On click of skip it will redirect user to next screen – General Health

8. General Health – On this screen user will select his/her general health things from categories – General , Skin, Eye/ears/nose/throat

• Patient can select multiple checkboxes here and can save it
• Patient can skip this also – On click of skip it will redirect user to next screen – “Tobacco”

9. Tobacco – This screen will show up after General Health

• It allows patient to select checkboxes of “cigarette use” (Single selective)
  On selecting (quit and Current smoker ) it will show custom fields
• Quit – Date and Age
• Current smoker – Packs per day and age
• Other Tobacco use – Allows user to select other tobacco consumption products he/she did before or doing right now
• On click of save it will store all details and redirect user to next screen
• User can also skip this and it will redirect user to next screen – Alcohol

10. Alcohol – This screen allows user to add Alcohol cosumption

• On No it won’t append other fileds
• On yes,0 -1 time/month , 2-3 time/ month and every week it will append other fields ( it is mandatory to add data in (each week, how many)
• User can fill all details and on click of save it will redirect user to next screen
• On skip it will allow user to move to next screen -Drugs

11. Drugs – This screen will appear after alcohol screen and user can skip this screen too or add value by selecting checkbox (single Selective)

• User needs to upload – His/her insurance card image
• Insurance provider – It allows the user to select insurance from dropdown and it also allows user to select custom insurance provider by adding its name manually (Click on other)
• Member id – Needs to be filled (Insurance member id )
• SGroup number – Needs to be filled (Group number of insurance)

• This screen will appear after adding insurance card
• It is not going to show these categories if ( Patient location nearby doesn’t have any doctors under 50 miles range)
• Speciality – It will display the specializations selected by doctor from their portal
  Below speciality it shows (2 specialists ,3 , 1 etc) it depends upon how many doctors have selected that speciality
• Search – User can search “specializations” if in case number of specializations are more in number
• On click of any specific speciality it will open another screen (Primary care physician)

2. Primary care physician (Any speciality selected) – It will allow user to change it’s location too

• On click of Map icon at top right corner it will allow Patient to view doctors in map under 50 miles
• This allows patient to view Dr. name , fee, address and its reviews

1. On selecting doctor it open Doctor details with its bio and doctor’s location on map with book appointment button

2. Book Appointment – It shows slots of doctor in which date doctor is available on basis of date and time

• Reason – It allows patient to add any text along with booking of appointment
• On selecting time slot and clicking on book appointment it will redirect patient to payment screen

3. Payment – Payment are for two types – card and Insurance

• On selecting card it allows patient to add card details and use it for booking appointment
  User can also save multiple cards for payment
• Insurance – User can user insurance card too as a payment and can store multiple insurance cards
• Only one of these payment way can be selected then click on make payment , It will show popup

3. Thank You Popup – this will display popup saying thanks for booking and with doctor name + date and time slot Patient selected for the appointment

• On click of okay it will redirect patient to – Appointments screen

3.Appointments – Upcoming -This will display the appointments which are not accepted yet or rejected yet

• Appointments which are getting accepted or rejected will display notification (If notifications are toggled as on from settings)
• If appointment will be accepted it will show chat icon and cancel icon And in pending appointment only cancel icon
  If user will click on chat icon it will redirect user to messages screen and if user click on cancel it will ask for confirmation and then User can cancel that appointment
• Canceled and completed appointments will move to “Past Appointments” tab
• Once appointment is accepted and completed by doctor it will show button “complete at user side”
• On click of that button it will release the payment to the doctor which will deduct 10% by admin from the doctor fee.
• On click of “complete” it will show the screen to rate the appointment and add review
